Level I QC Data Package (Standard):
- Client name and address
- Date sample received
- Sample description of ID number
- Date of sample collection
- Date of sample extraction or prep
- Date of sample analysis
- QC Surrogate Recoveries (Organics)
- Method reference data
- Chain of Custody
- Analyst's Initials
- Sample analysis data
- Case Narrative (for any discrepancies)
- Report summary page with Authorized Signature
Level II QC Data Package:
- Level I Data Package
- Method Blank results
- Laboratory Control Spike Summary and Duplicate (Where applicable)
- Matrix Spike/Matrix Spike Duplicate (MS) - Upon request
- Duplicate
Level III QC Data Package (Additional Fee):
- Level II Data Package
- GC/MS Tune - Where applicable
- Initial Calibration (IC)
- Continuing Calibration Verification (CCV)
- Spectral Interference Check (Metals)
- Internal Standard Area Summary
Level IV QC Data Package (Additional Fee):
- Level III QC Data Package
- Raw Data - Including instrument sequence logs, data sheets, chromatograms, spectra, extraction logs, and digestion logs.
- Additional Data Available Upon Request
If a QC Data Package is requested after the final report has been issued, a 10% surcharge will be added to the original invoice along with the QC Data Package cost.
